<p> Leading global public health experts and NGO and business leaders will convene for two days in Lexington, Massachusetts to identify innovative solutions for improving the treatment of multi-drug resistant tuberculosis (MDR-TB), which kills more than 150,000 people each year.</p>

<p>The goal of the Innovation Summit, jointly organized by The Lilly MDR-TB Partnership and the Stop TB Partnership, and facilitated by Innosight, is to find solutions for identified barriers to treating the disease. </p>
<p>&#13;</p>
<p>MDR-TB is a form of tuberculosis that is resistant to the drugs most commonly used to treat the disease. It afflicts more than 500,000 people worldwide with some 80 percent of cases occurring in India, China, Russia, and South Africa. </p>
<p>&#13;</p>
<p>Due to drug resistance, MDR-TB must be treated with second-line drugs (SLD). Because of a range of challenges, however, fewer than 20,000 patients are treated each year with quality-assured SLD drugs.  The summit will use Innosight’s solutions development process to focus on 14 critical SLD supply chain barriers from the viewpoint of different stakeholders. </p>
<p>&#13;</p>
<p>In attendance will be more than 35 experts from a variety of organizations dedicated to treating and eradicating MDR-TB, including the World Health Organization, the Centers for Disease Control, Partners in Health, the Global Health Delivery Project at Harvard, USAID, Wal-Mart, IDA Foundation, Eli Lilly and Company, Massachusetts Institute of Technology, and others.   </p>
<p>&#13;</p>
<p>Said Tracy Sims, Vice President, Lilly Foundation, “high-quality second-line drugs for MDR-TB exist, but a variety of barriers cripple the supply chain and result in an unacceptably high number of patients who go untreated. In bringing some of the most talented and committed experts in MDR-TB together at this summit, we believe we can make great progress in developing actionable solutions to break down these barriers, improve access to treatment, and save lives.”</p>

<p>Following the summit, participants will seek to identify methods to test and implement the identified solutions.</p>

<p> Bernstein Liebhard LLP, a nationwide law firm representing clients with metal-on-metal hip implants, is pleased to report that on April 25, 2012, the European Parliament’s Environment and Public Health Committee unanimously passed a resolution seeking to create a medical implant registry, which will allow health officials to implement measures to help track failure rates of  dangerous medical devices, such as metal-on-metal hip implants.*  In addition to creating a medical implant registry, the resolution also contained recommendations to  keep patients safe from dangerous medical devices, such as:  vigilant monitoring, product traceability, and establishing a pre-market authorization system.  The case of defective metal-on-metal hip implants and other dangerous medical devices “have shown a failure of the current system of certification of compliance . . . as well as of the controls of the notified bodies and their surveillance by national competent authorities,” the Members of European Parliament (“MEPs”) said. </p>
<p>&#13;</p>
<p>“Similar criticisms can be made of the U.S. system for regulatory approval of certain medical devices,” said Bernstein Liebhard LLP partner Felecia L. Stern.  According to Stern, “Patients with implants should be entitled to information concerning the performance record of those implants.  If there is a significant failure rate, as there was with the DePuy ASR metal-on-metal hip implants, that information must be made public—in real time.  If this country implemented a medical implant registry it would go a long way in helping save patients from the disastrous effects of defective and dangerous medical devices.”  </p>
<p>&#13;</p>
<p>European Parliament Calls For Strong Measures To Avert Future Metal-On-Metal Hip Implant Disasters</p>
<p>&#13;</p>
<p>According to the resolution, European Union legislation to monitor dangerous medical devices will be revised this year.  MEPs hope to improve the transparency and raise the criteria for checks, inspection, market surveillance and information sharing of adverse effects from dangerous medical devices.  One aspect of the resolution aims to strengthen traceability and coordination of devices such as metal-on-metal hip implants, which will make it easier for patients and health care professionals to report adverse events.  The resolution calls for a pre-market authorization system for dangerous medical devices to help patients be aware of the devices’ risks.  The system will allow information to be exchanged so that it will be easier for patients to be notified of defects; it also includes a protocol for patients to obtain compensation if they are injured.  Finally, the resolution suggests creating a single European database to bring together information about medical devices on the market, including device registration, surveillance, clinical investigations, notified bodies and EC certificates issued. </p>

<p>Dangerous Medical Devices Such as Metal-On-Metal Hip Implants Lead to Litigation</p>

Benzodiazepine use was associated with a substantial increase in mortality, and this was attributable to suicidal deaths and to nonsuicidal deaths. In contrast, the use of an antidepressant or several concomitant antipsychotics was not associated with an increase in mortality.</p>

<p> Parker Waichman LLP, a national law firm dedicated to protecting the legal rights of people injured by defective drugs, warns that prolonged use of proton pump inhibitors (PPIs) has been associated with serious side effects, including  bone fractures and hypomagnesaemia (low magnesium levels). This week, the U.K.&#8217;s Medicines and Healthcare Products Regulatory Agency (MRHA) warned patients and doctors there of the possible association between PPIs and either of these side effects.   The U.S. Food &amp; Drug Administration (FDA) issued a similar warning regarding hypomagnesaemia in March 2011.  The FDA ordered the makers of prescription PPIs to add warnings to their labels regarding the risk of fractures in May 2010.</p>
<p>&#13;</p>
<p>PPIs inhibitors work by reducing the amount of acid in the stomach, and are approved to treat conditions such as gastroesophageal reflux disease (GERD), stomach and small intestine ulcers, and inflammation of the esophagus.  The drugs, which are available in prescription and over-the-counter versions, are sold under a number of brand names, including:&#13;<br />
</p>
<p>&#13;<br />
     Dexilant (dexlansoprazole)&#13;<br />
     Nexium (esomeprazole magnesium)&#13;<br />
     Prevacid (lansoprazole) and OTC Prevacid 24hr&#13;<br />
     Prilosec (omeprazole) and Prilosec OTC&#13;<br />
     Protonix (pantoprazole sodium)&#13;<br />
     Vimovo (esomeprazole magnesium and naproxen)&#13;<br />
     Zegerid (omeprazole and Sodium bicarbonate) and Zegerid OTC
<p>This week, the MHRA warned that studies on a risk of fracture associated with PPIs suggest there may be a modest increase in the risk of hip, wrist, or spine fracture, especially if PPIs are used in high doses and over long durations (&gt;1 year). The increased risk was observed mainly in elderly patients, and it is possible that other risk factors contribute to the increase in risk.   According to the agency, two meta-analyses of studies suggest the risk of fracture is increased by 10–40% above baseline. The primary studies in these analyses have varied in the extent to which they have adjusted for other potential risk factors for fracture, and use of calcium or vitamin D, the MHRA said. <p>In May 2010, the FDA announced new warnings would be added to the labels of prescription PPI after its review of several epidemiological studies found increased risk of fractures of the hip, wrist, and spine with PPI use. Some studies found that those at greatest risk for these fractures received high doses of PPI or used them for one year or more. The majority of the studies evaluated individuals 50 years of age or older and the increased risk of fracture primarily was observed in this age group, the FDA said.[fda.gov/Drugs/DrugSafety/PostmarketDrugSafetyInformationforPatientsandProviders/ucm213206.htm]</p>

<p>According to another MHRA warning issued this week, a review of case reports described in the literature or reported to regulatory authorities in Europe suggests that PPIs may cause hypomagnesaemia. Some cases occurred after 3 months of PPI therapy, but most occurred after 1 year of treatment.  Symptoms of hypomagnesaemia may begin insidiously and be overlooked. In most case reports, hypomagnesaemia improved after magnesium replacement and discontinuation of the PPI. [mhra.gov.uk/Safetyinformation/DrugSafetyUpdate/CON149774]</p>

<p>Hypomagnesaemia occurs when there are low levels of magnesium in the blood.  Common symptoms of the condition include:</p>
<p>&#13;</p>
<p>     Abnormal eye movements &#13;<br />
     Convulsions&#13;<br />
     Fatigue&#13;<br />
     Muscle spasms or cramps&#13;<br />
     Muscle weakness&#13;<br />
     Numbness
<p>If untreated, hypomagnesaemia can lead to cardiac and/or respiratory arrest and death.</p>
<p>&#13;</p>
<p>In a Drug Safety Communication issued in March 2011, the FDA announced that it had mandated that information about the potential risk of low magnesium be added to the WARNINGS AND PRECAUTIONS sections of the labels for all prescription PPIs.  According to the agency, hypomagnesaemia has been reported in adult patients taking PPIs for at least three months, but most cases occurred after a year of treatment. Approximately one-quarter of these cases required discontinuation of PPI treatment in addition to magnesium supplementation.
